In our environment we are not creating deployment packages for commercial release, just for deploying our in-house developed applications within the enterprise. That said we will be deploying to a fairly large number of workstations spread out across several locations.

I've always used VS deployment--it can be a little flakey, but once you know how to get around its few "nits" the deployment works well. I haven't played with InstallShield recently, but it's always been a solid package.
