There needs to be some reform, but the plan out there isn't it.
I agree with all of the points you listed here.
The second major cause of high health care costs is what's being charged for medications and medical devices. Why? Because the FDA requires far more testing than is required in the rest of the world. This costs the drug and device companies billions of dollars in extra R&D and that's passed on to us. Congress should overhaul the FDA and make it easier to get these things approved.
Actually, I believe that the cost of drugs would be reduced if the life of a patent was increased. R&D costs are huge, so the drug companies have a very limited amount of time to recover their costs before the generics start getting manufactured. If they had more time to recover their R&D costs, the initial cost of the drug would be lower.
