Two: it was after a disaster so everyone experienced hardships in addition to those they already had on a day-to-day basis, three: Kentucky was and is one of the poorest areas in the country. There is a lot of sparsely populated areas there and in some areas, they choose to remain separate from much of the other areas (like some mountainous areas in North Carolina where the locals don't want interference). However, for that picture, it was a disaster in a populated area but it was also back in 1937.

In addition, we have some 'pockets' where folks intentionally stay 'removed from civilization' and sometimes, it is just a matter of the distance and services not available (not by choice in all cases). Thanks for the explanation Tracy. Can you imagine that such a photo can be abused by people who want to 'prove' that the USA is a hypocritical society?! Due to the internet, photos and stories from long, long time ago get revived while the new people do not know the true story behind things.
