A medical form has 3 memo fields for the user to complete:
Problem Actions Requested Actions Completed

The user needs to type in details for the above but the problem is for each problem the data entered in Actions Requested and Actions Completed needs to be on the same line as in Problem. This is OK until the user scrolls down the Problem field and then tries to scroll down the other 2 fields, keeping the data entered in them properly aligned against each Problem entered.

Is there a way around this? Can you have a single Memo field with 3 pre-set columns for the user to enter data into?
