What I noticed is that you do not have the latest version of Excel 2003 installed. The latest version is SP 3. Additionally, I would re-install the converter for Office 2007 files after installing the service pack. So,

Service pack 3 first:

then the compatibility pack again:

XLSM and XLSX are different files. XLSM does include support for macros. The person who is sending you the file would need to send you an XLSX file instead of the XLSM file.
