I'm a little puzzled since you write that your programs has worked earlier. In order to run a form, you must do two things
Do form yourform
Read events
Without the last line, your form won't start since step no 1 only "prepares" the form while step 2 actually "starts" it. From the command window you don't need Read events, but you definitely need it in an exe. I'm not 100% sure about an app though, only 55% :-)
