I was unable to find the respective ActiveX in Tools>Options>Controls>radio button ActiveX controls for some reason. I do have Microsoft Office 2010 installed on my box and use Outlook as my email client. What options do I have have to recover this form ? I don't mind deleting old ActiveX reference and putting new one, whatever it is. Surely, I don't want to install ( and I can't ) old versions of Microsoft Office.

I am not sure the problem is with Microsoft Office 2010, on my box I also have 2010 installed and the ActiveX is shown in the grid on page `Controls´ of form Options.

It seems you miss OUTLCTL.DLL which you may download at: http://activex.microsoft.com/activex/controls/office/outlctlx.CAB
