The serviceentrydate field is little-used, so there is a high probability the bound date field in that textbox was empty. It might be worth trying to start the form with the record pointer elsewhere on the off chance that had an effect.

Not that I can see off hand. But the bitmap = off is worth a shot. Since it's my (administrator) profile on all servers, the user printer config should be the same in all cases, but that's another thing to look at.
