A couple of things come to mind
(1) Meetings - when there is a problem to solve, people come with possible solutions.
When a common ground solution is finally picked people have the tendancy to believe the problem is solved.
It isn't until the work and the follow up for the solution have been done
(2) There are always explanations ( aka excuses ) why one has not been able to do something
The real art is to tell how you have succeeded in spite of the problems in the way
