The way I use it, and the way I've seen it used:

- Yes, it's approval/agreement
- Depending on the tone (of voice) there *may* be some sarcasm, but only in the sense of "that's stating the obvious". It does not mean or imply disagreement. In text, adding an exclamation mark ( "No kidding!" ) is sometimes considered to be sarcastic in that manner

If it's used as a question ( "No kidding?" ), that's most often used to question, but not necessarily disagree. Similar to "Are you sure?" or "Wow, that's unusual/hard to believe".
