I had to work with MTOM on a recent project and we ended up building the SOAP envelopes by hand. We actually started using .NET with WCF initially but found a number of compatibility issues and tweaks that were easier solved using manual creation of hte buffers. However, we did have a working SoapUI trace, which is crucial if you go down this path.
Since MTOM is basically mime data with raw binary data it's not terribly complicated to write out the MTOM data to send. Parsing it is a bit more of a problem.
