Hi Rick, thanks for your feedback.
I've finally could test a MTOM web service with binary attachments (zip), and it worked ok with MSXML 6.0 using the property responseBody te receive the binary data as mime, like you said. I have created too the Soap envelope, nothing complicated copying the using SoapUI generated one.
The only test remaining for me is the upload of an attachment using a MTOM service, and I think is doable.
