You should probably not set DLLs to single-use instancing, but rather multi-use. With single-use, you will only get 1 instance of the object created and then an error occurs. In addition, MTS requires multi-use. Single-use is more appropriate for EXE servers.

I'm not sure if this solves your problem, but its probably a good place to start in trying to isolate things further. Let me know what you find.
