The politicians are already deeply in the game.
Medicare and Medicaid - at least among the people I know- are the primary insurers now.
I don't see what would be wrong with letting people like my children buy into medicare, rather than having to finance the profits of insurers that add no value.

Insurers make more profits as medical costs rise. Taking away that bias won't cure the cost problem, but at least that influence will go away.

The ultimate guardian here is the MD.
The system can only succeed if the MD is doing what's right for the patient, and nothing more.
As consumers, we have a duty to be on the lookout for MD's who shill for big pharma, prescribe needless tests, etc. but ultimately there is no substitute for an ethical, competent MD.
When we look at spiralling costs then, we have to look first at MD's.
