I don't know if it's still on the market, but we used WordPerfect's InForms to do what you're talking about for a while; the form filler can be set up to read a Fox 2.x-style .DBF for input. We also tried FormWorx (we really liked it, but the original version wouldn't run under Win9x or NT, and at the time InForms cost about half of what FormWorx for the 32 bit environment cost.)
If it's still on the market, Corel would probably be the vendor at this point.
