Comment out all of the cvode except the line that errors and then try it. If it errors then there is something with the table, perhaps an index that refers to another table. If it does not error then add lines back in one at a time and try it again until it errors, the last line you added is the likely culprit.

You can also copy the lines out to a prg and try it there without any form running.
