I have found the problem with MAPISP32. My system was a clean install of Win98 (no other OS had been on it previously). In this case, this is significant. MS KB article Q230321 provide the answer, at least partially. The invalid page fault is now gone, but I can't get Outlook to retrieve new mail. I've deleted the ccMail service, and just finished deleting the Notes service. I've attempted to re-install it once, but it blew up. I'm going to try again.

BTW, this is just one more case of the utility of an MSDN Library Subscription. I didn't have to pour through the MS site to find the answer. Anyone who isn't taking advantage of the free one year subscription is plain missing the boat.
