Hi Simon,
Well, my solution may be too simple. I needed a way for team members to send me anonymous feedback on my leadership style. I have a resume site that is hosted on FreeServers.com. They have a form e-mailing service that is free when you sign up for hosting. If he doesn't mind advertising banners, he can do this for free. Otherwise, I think I pay $7 per month to keep the banners hidden.

You can see an example of the form in action at my web site. www.SmithLeadership.com There is a paperclip at the bottom right of the sign on screen that will take you to the form. There are many things that you can do with the form. Mine is rather bland. You can do anything with the form that you could do with a regular form. It's the address SUBMIT code that makes it special.
