So I was wondering what other suggestions people might have to collect this data and send it to him. The system needs to be simple to use since many of the students are in public school. The more automated the better I think.

Every web hosting site I've ever seen has a built in way to enter data on a form and send the data to a recipient via email. You can get the site hosted on about any hosting service there is and take advantage of the built in mail forms. My site is hosted on www.digitalspace.net - they're not fancy or flashy, but they are reasonably reliable and extremely inexpensive - $3/month.
