Every web hosting site I've ever seen has a built in way to enter data on a form and send the data to a recipient via email. You can get the site hosted on about any hosting service there is and take advantage of the built in mail forms. My site is hosted on
www.digitalspace.net - they're not fancy or flashy, but they are reasonably reliable and extremely inexpensive - $3/month.
