I've never placed an image of a form on a report but I've placed full-size watermarks across the page. Here's how to do it:

Place the image in the upper left-hand corner of the page header band, sizing it small enough to fit in the header. Then save the report, get out and hack the report file. To do this ...

USE reportname.frx
BROWSE

Look for the image (objtype will be 17). When you find that record in the report file, change the HEIGHT AND WIDTH to be the full size of your report. I think, but I'm not sure, that 1000 units = 1 inch on the report. It will probably take a few attempts at getting the size right. Once you've hacked the FRX, you can close the file and bring up the report writer again and continue with your report designing. It will look a little strange, so you'll have to play around with setting up your header, detail and footer bands and do a test print to see how everything looks.
