>Had another idea as I was wrestling with all this: scan sections of my form into three files, one header, one detail, one footer. Then just plop the 3 images into their respective areas of the report. Works pretty well, except for the 'empty' area below the last detail line and above the pgfoot area, but maybe that can be overcome.

Try it with only two scans. Scan the header and detail together and place that in the header like you are doing now. Then scan the footer as a separate scan and place that in your footer.
