For the host name use localhost ( spelled just like that NOT! i mean NOT! GOC) thats it. You can also try your local ip address i.e. 127.0.0.1 unless you have a constant IP adress or are on a network then use the machine's local IP address to see what that is click start-run- and type winipcfg and click OK this will give you your local IP address.

I am assuming you are using Windows 9.X if not I HIGHLY recommend you download the latest NT Option pack here..

The name is misleading it will run on Windows 9.x as well, you just can't install some of the features included. If you do make sure you grab the Microsoft Data Access Components (MDAC) as well at the following address...
