I would think a date would be a minimum requirement as well. In fact, if you aren't supplying a date, you might want to check if the transactions maybe did get imported and are sitting at the top of the register. If the register's in date sequence then blank dates would sort to the top, and if you're looking for them at the bottom they wouldn't be there. This happened with a client of mine one time.

Minimum required fields aside, you might still need to use all the field headers in the header records, and use placeholders (tab characters) for the unused data fields in the detail records. FWIW I always create the import files this way and they always work (at least so far).

So this suggests that the file is being processed correctly. Hmmmm.... the only other thing I notice is that on my file (again, this is the QB 2000 spec) there are two amount fields in the split record, amount and price. Maybe both are needed?

Well, no big surprise there. I'm sure we can figure this out without them, though.
