Ouch. Seen a couple of .50's blow up when the headspace wasn't set right, but never seen a slide come off. Easy enough, though.
I was at a boat show not too long ago, and the local recruiters were out with a river patrol boat display including the weapons. One of the things they had was a new HK .45 - really massive pistol. The Colt may be gone, but the cartridge is still being used.
