I heard a story about this in my first programming class (mid 70's COBOL). Seems that after five or more years of operating properly, this system started generating an error message "CHYNYOYO." The programmer was long gone and the documentation had been moved "somewhere." The documentation was eventually found and after several hours of reading miles of code they found
