To anyone reading the above thread and trying to make sense of the "recipe",
the content between the quotes did not show up. The content for the name should be: a quote, a less than, TONAME:name, a greater than, a quote.
The content for the phone number should be:a quote, a less than, TOFAXNUM:faxnumber, a greater than, a quote.
