I'm reasonably satisfied with the way Hotmail handles it

Any mail from someone in your address book is passed to your inbox.

You also have a "Safe List" which I use for newletters, support groups, etc.

Everything else gets passed to a Junk folder.

The biggest problem is that the Junk folder counts against your total space quota. If you've been as careless as I originally was about giving out email address, you constantly have to monitor and clean out the Junk folder since all the spam can chew it up.
