It only happens if while in the field I press CTRL+INS which causes the following events
KEYPRESS
INSERT
REFRESH grid
field.LOSTFOCUS
--- second replace command...

Have you tried setting focus off the grid and onto a different object on the form before issuing the INSERT command? That usually fixes the problem for me.
