Capture a query that hits the table using Profiler and then run it in QA to see the showplan. Maybe you're missing some indexes?

A few hours later somebody deleted a record from the table and right after that we ran the query again and the query time went back over 5 minutes, we don't know if the delete had something to do with it or maybe in between that time the performance was degrading without us knowing but after this I went to check the rows and it was back to 0, very strange.
