Hi Cathy - thanks for replying and sorry! - I didn't realise I missed all that detail!

Yep the second page is a repeat of the first page but instead of the fax details at the top, it's laid out like an order form (so yes this is additional information although it's not drawn from tables...). The detail lines are the same as on the quote with the addition of "boxes" so that the person placing the order can specify the quantity of items required.
