Albert, since you are in Canada it may not pertain to you but here in the states you can actually create legal problems using any form however precise that has not been pre-approved by the state department of insurance (or board of insurance depending on the state). We submit the forms to the carrier and to the board and once approved use them. When we purchase the forms from ACORD they are already approved.
