we all use MS HTML Help Workshop for creating of CHM.
Ussually I use word to create pages(documents) and save them in HTML format
Use a XSL to describe tree of documents and topics, references, ect. And made a small VFP program which build CHM from this things.
WestWind Help is done in similar way
