We've successfully used TAPI3 in c#.net to control a Nortel i2550 SoftPhone Headset in conjunction with a Nortel Symposium Server.

The code is something of a Black-Art on the 'Net, we had terrible trouble finding help. And the system is still very fragile as server configuration seems tricky to maintain.
